What Are All-on-4® Dental Implants?
All-on-4® is a dental implant procedure for one or two arches of artificial teeth that provides greater bite force than dentures.
All-on-4® dental implants may be an option if you are missing multiple teeth, have severe gum disease or tooth decay, or cannot otherwise have your teeth restored.
The All-on-4® procedure involves replacing your teeth with an arch of long-lasting, artificial teeth. These teeth are supported on four strategically placed dental implants—two at the front and two at the back in the upper or lower jawbone. This allows the new arch of teeth to be secure while also being less invasive than some traditional full-mouth dental restoration options.
At Next Smile Brisbane, we follow the clinically proven Malo Protocol for the All-on-4® procedure. Pioneered by Dr. Paulo Malo, the creator of the All-on-4® concept, the Malo Protocol is a two-bridge treatment process with a 98% success rate.
In the first stage, any remaining teeth you may have are removed, and the dental implants are placed. You are then fitted with your first set of ‘provisional’ teeth. You will wear these teeth for three to six months as your jawbone fuses with the dental implants. This also allows you to get used to the appearance and feel of the teeth and make any adjustments for your final set.
In stage two, you receive your final set of long-lasting teeth, crafted onsite at our Next Smile Australia Labs and made from the highest quality Nobel Biocare components.

Introducing Dr Tony Rotondo
Dr Tony Rotondo is a registered specialist in Prosthodontics with over 30 years of expertise. He was the first dental surgeon to bring the All-on-4® treatment to Queensland and has been placing dental implants since the early 2000s.
Dr Tony Rotondo has specific interests in dental aesthetics, dental implants, ceramic materials and interdisciplinary dentistry. Dr Tony Rotondo is a product evaluator for dental companies and presents numerous continuing education programs nationally and internationally.

After graduating from Saint Mary’s University in Canada with a Bachelor of Science degree, David travelled to Australia and completed his dental degree at the University of Queensland in 2006. Since graduating, he has completed several continuing education courses, including a Mini Residency in Prosthodontics and a Mini Residency in Implant Dentistry. David also taught dental students in their final year at the University of Queensland for two years.
Working alongside a prosthodontist for fifteen years has allowed David to expand his knowledge of performing complex rehabilitations involving crowns, bridges, and dental implants. Over the years, he has restored thousands of dental implants and has engaged in All-on-4® restorations for over ten years.
Outside of dentistry, David has obtained his private pilot license. He enjoys spending time with his family and friends and sneaking back to Canada when possible.
Sahil graduated from the University of Otago (NZ) with a Bachelor of Dental Surgery in 2009 and then obtained thirteen years of experience as a dentist in a private practice. Before moving to Brisbane in 2018, he owned a dental practice in Canberra for six years. He also mentors recently graduated dentists and teaches courses to other dentists on dental aesthetics, veneers, and rubber dam application.
Sahil is very passionate about his work. Dr Sahil Soni strives to create trusting relationships with his patients, prioritising honesty and hard work in every interaction. His dedication to restoring smiles and boosting confidence underscores his genuine enjoyment of dentistry.
Outside dentistry, he enjoys reading, hiking, photography and playing squash.
Po completed a Bachelor of Dental Science with honours at the University of Queensland in 1999. After several years in private practice in Brisbane, Po followed his interest in prosthodontics and received specialist prosthodontic training from the University of Melbourne. He completed his Doctor of Clinical Dentistry in Prosthodontics in 2004. After relocating to Sydney in 2006, Dr Po established Sydney Pros Dental (when it was known as the Hunter Street Surgery).
Po has a special interest in implant dentistry and aesthetic dentistry. He enjoys coaching other dentists on best practices both locally and interstate and has enjoyed teaching undergraduate dental students for the past sixteen years. He is a member of the Australian Prosthodontic Society, the Academy of Australian and New Zealand Prosthodontists, and the Australasian Osseointegration Society. He also regularly donates his time to charity dental services, including the National Dental Foundation and the Red Cross.
Outside of dentistry, Po is a dog lover. He also enjoys watching movies and tries to travel a few times each year.

Casey has been Next Smile Brisbane’s Treatment Coordinator for the past 5 years, she has over 13 years of experience in the dental field, her experience as an exceptional dental nurse for dentals specialists in combination with her strong administration skills and outstanding customer service makes her a favourite of our patients. She is a very caring person and is currently on maternity leave.

Patrice has been in dentistry for ten years and has worked for Dr Tony Rotondo for over eight years. Initially, as a dental assistant, she was involved closely with All-on-4® patients during the restorative side of their procedure, which gave her a wealth of knowledge. She loves being able to chat with patients about how this procedure can improve their lifestyle. She has also worked as a client relationships manager and is now the Treatment Coordinator at Next Smile Brisbane. In addition to her experience in dentistry, Patrice uses her creative skills to manage her own business: "Simply Patrice Art".

Dani was born and raised in Puebla, Mexico and moved to Brisbane in 2016. She graduated with a Bachelor's in Dentistry in Mexico and has been in the dental industry since her teenage years when she used to help her father, who is also a dentist. Dani has worked for Tony Rotondo for over seven years, starting as his dental nurse, then as a clinical coordinator, and now as a treatment coordinator. She has over a decade of dental industry experience and a deep understanding of dental procedures and treatments. She’s a skilled care provider with attention to detail, caring and empathetic when dealing with patients prone to anxiety. Outside work, Dani likes to read and listen to podcasts, loves fashion, and enjoys walking with her husband and beagle.

The main advantage of the All-on-4® procedure is that it allows you to carry out a full arch restoration without the need for bone grafting and to have immediate temporary teeth (immediate load).
It typically requires only one operation of shorter duration compared with other forms of full arch rehabilitation (fixed teeth) that may include bone grafting.
The Next Smile Australia method has a high success rate, 95-98% in most individuals over a 10-year period*.
It is easy to maintain your hygiene utilising dental floss, waterpick and brush.
You can select the teeth aesthetics that you want and most importantly, you have the opportunity to try the teeth for three months during osseointegration (healing phase) before proceeding to the full titanium/acrylic teeth.
If you break a tooth on your final prosthesis it is relatively quick and easy to repair.
Some people cope well with dentures, others do not. Dentures have been available for hundreds of years.
They work by adhesion in your mouth and the success of dentures may be dependent upon the amount of residual bone post extraction of your teeth. They may also be affected by the amount and consistency of your saliva. In most cases, as the length of time you are missing your teeth increases, the bone levels continue to shrink and may cause more difficulty in stabilising your dentures.
Dental implants and the All-on-4® treatment options allow the bridge of teeth to be fixed to the dental implants (foundations) and therefore provide improved stability compared with dentures. Once the teeth are attached to the implants, they have the same stability as your natural teeth but will feel different as all the teeth are connected together in a bridge scenario. There are no individual teeth in the All-on-4® treatment options.
As the teeth are attached to the implants you should enjoy a better outcome in terms of functionality and appearance. All-on-4® may provide you with an easier cleaning and maintenance regime, which is essential to their longevity. They are designed to function similarly to your own teeth and provide confidence that there is stability of the teeth when biting, chewing, smiling, laughing and playing sport.
Following your initial consultation with our experienced dental teams you will be provided with detailed treatment plans for your surgery and rehabilitation. Once you have made a decision on your treatment options you will be provided with a timeline for your treatment.
All-on-4® generally has a 1-3 hour operation time, depending upon the number of jaws you treat and in most cases the temporary teeth are provided on the same day. In some cases we may need to insert the teeth on the second day, within 24 hours of surgery.
Dental implants are generally a long-term solution for your dental restoration. They do not last forever and the length of time they last may be affected by a number of circumstances.
Long term results may be dependent on your oral health habits, lifestyle and medications that you are taking now or in the future and your current and future medical health.
