How do Next Smile All-on-4® Dental Implants work?

All-on-4® is a medically advanced and proven* process that replaces your teeth only using four implants to support a new smile. The best part about the procedure? We aim to have your new teeth fitted within 24 hours of surgery, but usually on the same day.

At Next Smile Australia, we use the Malo Protocol, a two-bridge treatment process pioneered by Dr Paulo Malo.

In stage one, you’ll receive your first teeth, a ‘provisional bridge’ left in place for three to six months while your mouth heals. Once your bone has fused to the titanium implants, you’re ready for stage two. Stage two of the Malo Protocol is fitting your new teeth, the ‘final bridge’.

Who is suitable for All-on-4®?

It’s good to know if you’re suitable. Candidates for the treatment may include:

  • People who have lost multiple teeth due to injury
  • People who have lost multiple teeth due to tooth decay
  • People with advanced gum disease
  • People who cannot have their teeth restored
  • People who don’t see significant benefits from restoring their teeth

While good candidates don’t necessarily have an entire arch missing, they have many teeth that need replacing.

The criteria for the treatment are different than for dentures or single/multiple tooth implants. For this reason, candidacy is determined on an individual basis by your Next Smile Australia dental surgeon.

Frequently Asked Questions

What does All-on-4® mean?

All-on-4® is the name of the procedure invented by Professor Paulo Malo who established this advanced protocol for the treatment of patients who have dentures that they are unhappy with and want a better solution and for patients that have a terminal dentition from either dental decay or gum disease. The process involves only four implants to support an entire bridge of teeth.

Not all people with periodontal disease or dental decay require All-on-4® treatment. With many patients we can restore the teeth and treat the gum disease for many years. All-on-4® should not be considered a treatment of choice based on shorter treatment times or cost. At Next Smile Australia we believe we should not be removing good restorable teeth.

Are All-on-4® dental implants permanent?

We would hope that implants last forever but in reality they may not. There are many external factors that may affect longevity. By assuming they do not last forever, you are more likely to be better prepared for the outcomes.

Can All-on-4® implants be removed?

The dental implants are never removed, except when they fail! The bridge is the only removable component and is removed once a year to check and clean.

Do All-on-4® dental implants look real?

With all dental restorations, whether it be on dentures, implants or your own teeth we endeavour to provide you with a natural aesthetic appearance that is commensurate with your age.

Of course, many patients seek to have very white teeth and ultimately the appearance will be as you select. That can be achieved with size, shape and colour of teeth, and the display of teeth and/or gum that you see in your smile.

The benefit of the two-step Malo All-on-4® protocol is that you have your temporary healing teeth so you have a number of opportunities to review the appearance prior to completion of the final teeth both in the temporaries for 3 months and secondly the try in of the final teeth.

How many teeth are on All-on-4®?

The All-on-4® protocol allows for four implants and 12 teeth. The basis of All-on-4® is you surgically use the available bone.

It would be vary rare to have 14 teeth because the teeth would cantilever back from the last implant, causing potential fracture of the posterior implant and loosening of the abutment screws. Ideally you should have zero cantilever but the Malo Protocol allows for a maximum of two teeth cantilevered if required.
