All-on-4®

At Next Smile Australia, we offer the All-on-4® treatment concept.

What is All-on-4®?

The All-on-4® treatment is a dental restoration solution that offers a long-term, fixed alternative to dentures. All-on-4® involves replacing all teeth supported on only four implants to support your new, natural-looking teeth. Our innovative technologies and the use of superior materials help to overcome bone deficiencies and reduce the need for time-consuming and invasive bone grafts.

Compared to older treatment methods, All-on-4® not only reduces the number of implants required but increases the success rate with a proven success rate of 98%* and also provides for simpler maintenance and easier cleaning.

Dental implants - small titanium fixtures inserted directly into the upper or lower jawbone - have long been the bedrock of any restorative dental procedure. They are used to support a single tooth or ‘bridge’ of teeth. The same principle works when they are required to replace a full set of teeth (a full-arch rehabilitation), but there is no need to place an implant under every single tooth. During the All-on-4® treatment, only four implants are required and strategically placed, two at the back and two at the front of the mouth, to support the entire ‘bridge’ of prosthetic teeth.

All-on-4® two-stage process

Next Smile Australia follows the Malo Protocol, a two-bridge treatment process pioneered by Dr Paulo Malo, who invented the All-on-4® treatment concept. What does following this clinically proven protocol mean for you? A more predictable outcome* for your new smile.

Stage one involves your All-on-4® surgery, where you have the dental implants surgically placed and receive your first set of teeth, a ‘provisional bridge’ worn for three to six months while your bone fuses with the implants. Once your bone has fused, you are ready for stage two.

Stage two of the Malo Protocol involves you being fitted with your 'final bridge', your final set of teeth. Your new teeth are created by our Next Smile Australia Labs using genuine Nobel Biocare components.

The two-stage protocol aligns the All-on-4® treatment with the highest quality component technology from Nobel Biocare, designed by Dr Paulo Malo for use with All-on-4®. It is implemented by fully trained professionals with many years of experience. Many Next Smile Australia dental surgeons have been trained in the Malo Protocol by Dr Paulo Malo himself, and as such, we are the only All-on-4® provider in Australia personally certified by Dr Paulo Malo.

We understand that this may all sound extremely technical, but rest assured that when you come to Next Smile Australia, you will receive the highest levels of care, experience and technology supported by a proven protocol that we believe is one of the best you can find, delivered by one of the most professional and highly qualified clinical support teams available.

What makes All-on-4® different to traditional dental restoration methods?

Dental implants have long been the bedrock of any dental restoration procedure. Typically, they support a single tooth or ‘bridge’ of teeth. The innovative technique of using only four implants allows for maximum support and stability, which helps to improve the overall function and aesthetics of the teeth.

The implants can be inserted at an angle, which enables us to avoid sections of the jawbone of poor quality. As a result, the healing process is faster as there is no need for invasive bone grafting.

With Next Smile Australia All-on-4®, our aim is to provide your temporary (provisional) teeth fitted within 24 hours of surgery, but usually on the same day.

Next Smile Australia’s All-on-4® treatment’s minimalist approach means fewer implants and more flexibility to design the best-tailored solution for you.

The All-on-4® Evolution

Dental implants can be a good solution for tooth loss. However, for some patients that have lost multiple teeth or are wearing dentures, bone loss is a major issue in restoring the mouth to an optimal functioning level, previously, bone grafting was the only option requiring longer healing time and higher costs in treatment. The All-on-4® technique reduces the need for bone grafting, which can make the procedure faster, less healing time and more affordable for patients. The All-on-4® treatment has since become a popular and effective option for people who are missing most or all of their teeth and as a denture alternative.

To take down the barriers that kept patients from receiving the smiles they deserved, our mentor, Dr Paulo Malo, developed the All-on-4® treatment concept. The key to All-on-4® is the precise placement of only four implants in the upper or lower jawbone, positioned to avoid areas of low bone density. The implants placed in the back of the jaw are angulated/tilted, avoiding the thin or poor bone.

All-on-4® is an innovative solution that shortens and simplifies the process of complete full-arch rehabilitation. It has a higher success rate and omits the need for expensive and time-consuming bone grafting. As a dental restorative surgery accessible to more people, including those previously unsuited for traditional dental implant treatment, All-on-4® may allow patients to live their lives normally, chewing, biting and smiling without being self-conscious.

Frequently Asked Questions

Are All-on-4® dental implants removable?

Once implanted, All-on-4® dental implants and teeth are ‘fixed’. They can only be taken out by a trained dentist for regular maintenance and cleaning. It is important to remove the bridge of teeth regularly and check connection of the abutment screws between the implants and fixed teeth.

How much does All-on-4® cost?

Understandably, this is one of our most frequently asked questions. During an initial All-on-4® consultation at a Next Smile Australia clinic near you, we fully examine your dental health and if we establish that All-on-4® is suitable for you, then we provide you with a detailed treatment plan and corresponding costs.

The All-on-4® treatment costs $27,000 for a single arch. Not included in this costing is the anaesthetic fees which are billed separately by the medical provider. Those fees will be quoted to you prior to your surgery and are eligible for Medicare rebates.

For more details on pricing and payment plans, please visit our pricing page.

Are All-on-4® dental implants safe?

At Next Smile Australia, safety of our patients is our priority. The two-bridge Malo Protocol we follow has been clinically proven over 25 years, resulting in a 98% success rate*. We only use TGA approved dental implants and materials.

Even if implants are placed by experienced professionals, there is no known way to determine if your body will accept them. So, careful analysis of your medical situation may assist in determining potential success rates.

How do you clean All-on-4® dental implants?

The bridge of teeth can be cleaned by you on a daily basis. The regime will include use of a toothbrush, specialised dental floss and a water flosser. You will be given detailed instructions on how to carry out this regime at home and ongoing support to ensure you understand this process. You will need to have your All-on-4® professionally cleaned every 6 months.

How long does the All-on-4® procedure take?

Following your initial consultation with our experienced dental teams you will be provided with detailed treatment plans for your surgery and rehabilitation. Once you have made a decision on your treatment options you will be provided with a timeline for your treatment.

All-on-4® generally has a 1-3 hour operation time, depending upon the number of jaws you treat and in most cases the temporary teeth are provided on the same day. In some cases we may need to insert the teeth on the second day, within 24 hours of surgery.
